my tranny now? nice

Ok so now when im on the highway sometimes im at like 110 kmph, then if i give it more has engine just revs heavy untill finally it shifts hard. When im not giving it a lot of gas it seems to shift a lot smother, however when slowing down sometimes i feel a jump in the car... Any opinions are appreciated obviously you guys cant tell me how to fix a tranny but im just wondering what these symptoms sound like.. thank you.. time for a sonata soon i think the new ones are so sexy
